Michigan Rehab and Performance was founded on the idea that the client comes first. Healthcare quality is at an all time low. Many people pay for healthcare not just with their money, but also with their time. Time is the most valuable resource we all have.

Our goal is to help out clients live a high performance, pain-free life. If you like to do CrossFit, we'll help you stay healthy while you train. If you love tennis, we'll get you back on the court and keep you there so you enjoy quality time with your friends/team. If you're a runner and have aspirations of doing your first half marathon, we'll help you accomplish that goal!

We want our patients to know that we have their best interests at heart. We also want to be a resource for our community, coaches, athletes, and trainers around the world. We constantly put up new videos, podcasts, and blog posts to help our community stay healthy and perform at their highest level.

 

Jacob Centala, DPT

Dr. Jacob has been surrounded by sports his entire life, growing up primarily as a basketball and baseball player. In his freshman year at The Ohio State University, he tore his ACL, which led him to his first experience with physical therapy, and he was hooked. Between this experience and his recovery, he immediately dove deep into the fitness world and fell in love – primarily beginning with traditional weightlifting and recently transitioning into CrossFit. Ultimately, this led him on a path to graduating with a Doctorate of Physical Therapy from Wayne State University. After graduating, Dr. Jacob knew he wanted to help athletes and individuals get back to doing what they love, but he quickly realized that the modern healthcare system is designed to provide neither the time nor the care that many of these people need.

When Dr. Jake offered the opportunity to join MRP, it was a no-brainer. At MRP, he found a workplace where the company’s and clinician’s values and priorities were aligned and where we treated individuals who shared the same interests and passions. MRP is a place that allows its therapists to do what we’ve always wanted to do – treat those in need and get them back to doing what they love without having to jump through hoops or deal with any barriers.


Ryan Kilgore, DPT

Dr. Ryan enjoys working with various orthopedic diagnoses and implementing corrective exercises and manual techniques to restore proper movement patterns in patients of all skill levels. As a prior soccer player, Ryan enjoys working with youth athletes and trying to get back to field and court sports. He uniquely understands the demands put on our young athletes. Ryan believes physical therapy can be a place to have fun and utilize education and exercises to help patients achieve their functional goals. In his free time, Ryan likes lifting weights, playing soccer, camping, and hanging out with his family!


Jake Blackmer, DPT OCS

Founder and Owner

Dr. Jake Blackmer is a physical therapist who focuses on sports medicine. He is the founder of Michigan Rehab & Performance (MRP), which aims to assist athletes in identifying, rehabilitating, and understanding their injuries while enabling them to continue training at high levels.

Dr. Blackmer's journey into physical therapy was not just a career choice, but a personal passion. His time wrestling at Olivet College and his love for CrossFit, which led him to obtain his CF-L1 certification and start coaching, gave him a unique insight into the world of sports. This personal connection to sports is what led him to pursue physical therapy at Wayne State University.

After completing his education, Jake was eager to assist athletes but soon found that most rehabilitation clinics were primarily concerned with profit. Frustrated with the lack of understanding of athletes' needs, demands, and mindsets within the medical system, he created a solution and established MRP.

During the development of MRP, Jake also developed a passion for outdoor sports such as cycling, running, and archery. He pursued advanced training in these areas and can perform bike fits and gait analysis for his athletes.

At MRP, athletes receive one-on-one care from a doctor of physical therapy who not only understands their sport or hobby but actively participates in it. The team at MRP shares a passion for what you love, understanding that experience is the best teacher.
